Steps to Verify Eligibility
(Step 1) : Fill out your new patient intake form online. Make sure to click the insurance button and fill out all of the information [name, DOB, insurance carrier, medical record number, group name, and group number]
(Step 2): Schedule an appointment 3-7 days out online or give us a call at 9169298155. If you schedule online before filling out your intake form make sure you provide the necessary information [name, DOB, insurance carrier, medical record number, group name, and group number]
(Step 3) When scheduling an appointment online or via phone make sure to provide a phone number and/or email where we can contact you with your eligibility results [which will include yearly alotted visits, co-pay, and other insurance coverage information]
